[Templates] Including a BLOCK fails - TT looks for file

This has happened with several different blocks, so I am pretty sure it has nothing to do with the code in the BLOCK. I have tried to use PROCESS instead of INCLUDE with no success. Any thoughts on what is happening and if this problem can be eliminated?
What version of TT are you using and are you using any custom TT code? Does the exact code you gave us fail?
